- Codes C1241 Too low or to high battery voltage. C1256 Accumulator low pressure fault. C1391Abnormal leak of accumulator pressure. Checked pressure sensors. All checked good. Tests indicate a problem in the ABS Actuator assembly. Needs the actuator assembly replaced. There is a Service bulletin from Lexus fro a warranty extension for this issue.
